This survey is designed to focus on Spanish-speaking and minority individuals and collect data on pre-diabetes and diabetes awareness, challenges, community health resources, nutritional knowledge, access to healthy food, attitudes towards healthcare providers, and perceived barriers.

It is part of the INSPIRE-D project by ICNA Relief and Shifa Clinic, aiming to expand healthcare access and education to empower individuals to take control of their health and decrease new cases of diabetes.
